This short online form will be shared with Camp Medical staff ONLY to ensure a safe experience for your son and others at camp. It includes a few questions on vaccination status, allergies, and required medications. It should be completed by a parent.

This authorizes Troop 35 to disclose health information about your scout to medical personnel in the event of a medical emergency.  It should be completed by a parent.

This 2-page BSA form is a parent authorization and release for scouts to participate in various activities at camp. It should be completed & SIGNED by a parent. 

This BSA form provides us with General Health information, including immunizations and medications required, if any. It should be completed by a parent.

This BSA form is for a physical exam completed within the 12 months prior to the start of camp. It should be completed by a licensed physician (MD, DO), nurse practitioner, or physician's asst.

This BSA form is a release for dads to participate in various activities at camp. It can be completed by the Dad attending camp.

This BSA self-guided Youth Protection Training includes 5 modules and takes 72 minutes. When complete you'll get a certificate emailed to you that you can forward to camp.estar@gmail.com    Click on the link to the left to Login or create a free account; Then find and Click on “Youth Protection Training” under the Programs section. Choose “Mandatory - Youth Protection Training”. It should be completed by ALL Dads attending camp.
